Adding/submiting a file using a html online form

Hi,

I created a html template for a online form for a custom record. It all works fine, but I would like for the customer to be able to submit a file (picture) to the online form, but I can't see that option anywhere.

Is there a piece of html code for this or does it require scripting for the custom record?

The alternative is to redirect the client to a page where he can submit the file using a normal .asp request. But if I do this, is there a way of "tagging" the file so I know it was submitted by customer X, Y or Z?
