How do I add a signature field to an estimate, similar to what NetSuite does with its estimates?

At the very bottom (of the last page only, if multiple pages), I need to add:

I AGREE TO THE FEES AND TERMS OF THIS STATEMENT OF WORK:

________________       _____________________    _______________
Print Name                    Signature                          Date

The above signature indicates agreement with XX Company standard Terms and Conditions, and said Terms and Conditions shall apply to all orders generated from this statement of work.

What is the best method to make this happen?  Obviously it's possible, because NetSuite does it...

Any help is greatly appreciated!!
