FOB & Terms won't populate on custom form

I've created a custom form and have added separate fields for the FOB and  Terms. I know these are located in the body field, but I do not want them grouped with the other body information. Therefore, I have created additional field boxes for these two items, however the information from the transaction form does not populate into the custom .pdf form. Any suggestions?
