renaming column fields not holding

We are using a customized sales order form.  After some discussion it was decided, for clarity, to rename a column in the item section.  So, I opened the form for editing, went to screen fields>columns and made the change.  I saved it and went to a sales order screen to make sure it was good.  It was.  A few minutes later I opened an order screen to actually input an order and the column had reverted back to its old name.  This has happened 3 times now.

Any ideas?
