Online update creating duplicates

in our current month, and even in past months, we are seeing a mass up duplicate transactions.  First time, we matched them or recorded them...  all is good... then later, we are seeing the transactions matched, and Unmatched... 

to get to a previous month, we have to delete each upload in sequence to get to the offending upload.....  but now, we are seeing some in last September there were not there before...

Anybody seen this?  Solutions?
