Item - System Notes Crazy - UPC Code Unset

I am seeing tons of the following types of entries on my item system notes:

08/22/2007 8:55 am  [Employee]  Unset  UPC Code  123456002110

On one item, I have 2400 such entries.  Crazy thing is that employees are not editing items, and I don't think they have any permission to.

Every entry is Unset UPC Code with the current/correct value under old value, and BLANK for the new value.

Any ideas?
