Outlook Integration Crashing with Outlook 2007

I was using the Outlook Integration plug-in with Outlook 2007 and it was working just fine.  However, recently, Outlook crashed and reported that it was having trouble with the Outlook for Netsuite plug-in.  I disabled it and Outlook works fine, but every time I re-enable it, it crashes.

Outlook 2007 is the only Office app that I use on this PC (Windows XP).  I removed the Netsuite for Outlook plug-in and then reinstalled it.  Same issue - Outlook crashes.  I then uninstalled both Netsuite for Outlook and also removed Outlook 2007 itself.  I then reinstalled Outlook 2007 (and all subsequent updates) - Outlook runs fine. But when I install Netsuite for Outlook - it crashes.
