Item Fulfillment Form - Capture Package Dimension

We would like to add package dimension to the item fulfillment form. I looked at our form and can not see a way to access the Packages Tab to add a custom field. Has anyone been able to add a field to capture the data? We are not using integrated shipping with UPS/FedEx yet.
