Workflow Going Back to Initial State

Hello, Community! 😄

I created this EST & SO Approval Workflow:

Note that there's no transition from Pending Sales Head to Creation state. However, when we click the Approve button when it's in Pending Sales Head Approval, the workflow is going back to Creation state.

Here are the only actions on Pending Sales Head Approval:

The above issue is being encountered on both EST and Sales Order. This hasn't worked as expected before as I just created this Workflow for our new process. Please advise. Thank you in advance! 😄
