customized configuration for service logistics and subscription

Hi There,

we want cutomized Service request page to display the subscription related to the asset we have choose in service request and also shows the history of subscription for this asset.

Also we want to create susbcription from service request page, like in this picture attached.

Please guide how we can achieve this customization in service request page
