'That search or mass update does not exist' when submitting new online account form

We use SuiteCommerce standard.
When a customer fills out the customer online form, a new lead is created. It has been working without any issues until about two weeks ago, when we noticed multiple leads (2 - over 10 new leads, but there is no pattern) were created with the same information.
It turns out that when a customer submits the form, no confirmation page to follow. Instead the page remains the same except that it contains a message "That search or mass update does not exist". See screenshot below.
So the customer clicked the "submit" button multiple times until he/she gave up. But each click generated a new lead record in the system.
