How do I populate Custom Entity Fields when doing a bulk import?

I have created some custome entity fields that I will start using from here on out.

However, all the customers and leads I have in the past will have those fields blank unless I populate them. I would like to populate them all at once by doing a bulk lead import. My problem is, how do I set up the NetSuite Lead import csv template so that it includes my new Custom Entity Fields.
