Styling an online customer form?

I'd simply like to style my form to look like the rest of my website.  I couldn't find any way to do this so ended up actually building the form code myself and pointing the action of my form to the netsuite side and it worked.

Now there are two problems - I am not getting dynamic select options so if we change the form in our account, the changes do not show up on the page unless I recode the page.  Also I am not capturing the number of "requests" for the form.
