How many Organizations use Customer Centers?

We are considering whether to use a "Custom" Customer center for our customers who need to login to our system and then fill out medical information for our needs.  What I would like to know if how many people use the Standard Customer Center the way it is and then how many people use a custom version of the Customer Center.  Is there a way of Customizing the Customer center so that it looks nice?
