Online Form with Multiple Child Records

Hi - I'm working on an online employee application.  It's a custom record.  It's for Driver jobs, so ideally there would be several child records attached to the Parent record, for example Accident History, Licenses Held, Work History, etc.

Is there a way within the stock NetSuite to build an online form for the Parent Record (Applicant Information) that will also allow the applicant to enter information for those Child Records (Accidents/Licenses/Work History)?
