Dependent List Displays ID

I've created a dependent list on the Customer Record following the NS Help on Creating Multiple Dependent Dropdowns.  Everything filters properly, however on the customer record the "dependent" list only shows the ID of the custom record and not the specific filed I need to draw from.

Any suggstions on what I may have missed?
