Pulling Customer Info to auto-fill in an item description within a Sales Order email.

I hope I can explain this clearly:

What we have done:

1 - We have created a custom case form with fields asking for customer info.

2 - We have also created a Sales Order email that gets sent to the new customer upon order placement.

3 - And lastly, we have created an Description Item as part of a group of items.


What we would like to happen:

When a customer orders this specific product, they will receive the order email (as usual), but this particular item has a link to the custom case form that we have created.
This link within the item description on the sales order email should appropriately fill in customer info at the end of the link.
