Tracking Sales Rep changes

Is there a way to track when a Lead/Customer is re-assigned to a new  sales rep?

Some of our managers want to know which leads are being re-assigned, when they are re-assigned and to who. They would like to report on this information.
They were attempting to use the system notes, but the sales rep fields do not populate the notes.
