How would you create the item below?

We have an item we get from the vendor that can also be ordered with an optional add-on that is a separate part number, but actually becomes part of the primary item.  The vendor needs us to order it under the separate part number but obviously we cannot use a kit to combine because there will not be a physical item of the second item to pick.

Any thoughts how to accomplish this?
