Troubleshooting Email in WorkFlows

I have 3 states that send an email upon entry and 2 of them work fine, but 1 of them doesn't.

This one doesn't work, but it shows the correct person in the Active Workflow parameters for Current Approver.

Edit Send Email To: Workflow : Current Approver, Subject: You have a Time Off Request to Approve Entry

Not sure why it isn't working. Any tricks to this?
