Workflow doesn't work in netsuite UI

Hi,

I have a workflow when a new lead is generated. Workflow is working when a new lead was submitted via web form. But when entering a new lead via Netsuite UI, it's not working.

I don't see anything wrong with the transitions/conditions, but it's strange it's not working.

Any idea or suggestions is highly appreciated.
