Email through workflow on State Entry?

I have a few 'send email' actions in my workflow that are working fine.  One however, is not and I cannot figure out why.

Basically, it is about five states into my workflow, and it is triggered on entry of a state.

If it matters, the state that it enters into is from the previous state's field being changed from a specific old criteria to a specific new criteria.

Any idea why it is not working?
