Lead Nurturing Workflow sending only first state email

I have setup a lead nurturing workflow that send out a set of emails to leads on a certain interval.
I only got the first email even though the next state is after a delay of 30 minutes.
I have the trigger on field set to "On Entry" on the state that send the second email.
Tried changing it to Scheduled as well with no luck. Do you have any ideas on this?
