Advanced PDF/HTML Templates - Statement of Account

Hello there,
 
  Good day!
  I have a problem regarding the Statement of Account.  I need fields in my printout and I don't know yet how to solve this problem. I want to show Statement Date and Start date in my printout.
 
  Example :
  Billing Period :  From 7/2/2016 to 8/2/2016
 

Print Statement
