calculation on pick ticket
                    The company that did our implementation created a pick ticket for us that is supposed to include the item price and the extended amount for the item.  The field they are using is showing the value of the entire line and not the quantity that is shipping.  I changed the line on the advanced PDF/HTML template to be 
[CODE]Ext Amount: ${item.quantitycommitted * item.rate}[/CODE]
When that prints I am not getting a dollar sign in front which I would like. But the real kicker is I don't get a consistent number of decimal places. Can somebody help me format it so I always have 2 decimal places and with the dollar sign (if not possible for the dollar sign I can live without it).
