Creating a PDF form to use to Print Out a Customer Service Case

Greetings,

We would like to be able to print out a Case form to take with us to a customer's location to get their signature that the issue was completed.

I've not created a custom PDF form before, but it seems pretty straight forward. However, There doesn't seem to be an option to use Case records.

Has anyone created such a form for Cases? If so, what did you do?
