PageInit error when Online Customer Form opens in iFrame

I am experiencing a PageInit error when Online Customer Form opens in an iframe in Internet Explorer and Safari. Works fine in Firefox and Chrome. When I open the online customer form outside of the iframe, I do not get the error message.

Here is the error message:

An unexpected error occurred in a script running in this page.
PageInit(pageInit)
customform

JS_EXCEPTION
ReferenceError 'PageInit' is undefined

I need some help on how to resolve this. Thanks
