Roles?

Just curious as to best practices for roles.  Do most people customize every standard role (at least those roles you want to assign users to; even if you don't immediately change any permissions) right away and then assign users to those custom roles?  Seems that if you add users to the standard roles it's pretty much inevitable that you'll have to change some permission or other, then the user loses all customizations once you customize the role and assign them that one.  Is this correct, or am I missing something?
