PDF Mail Merge Template Examples?

Hello All,

I am trying to created my first PDF Mail Merge template and am not having a lot of luck.  I know how to create them in Word and place the CRMSDK tags, but I have never used Acrobats Form tool to create a template before.  Does anyone have some pointers or an exmaple they could forward/share?
