Surveys for customers

I would like to collect some data from some of our customers.  For example I would like to ask them what their invoicing preference is (email, fax, snail mail, etc)

I could compose an email to each customer but am wondering if NetSuite has any systems in place to conduct a survey that would take the customers to a form where they answer the questions?  Something more intelligent that just an email with questions?
