Return Authorization Form

Does anyone use any kind of form or link for their customers to use return authorizations besides using the customer center?  For example when we have small customers like ones that will only be maybe placing an order or two and then they need to return an item we would like to have them fill out some sort of form which then can get approved if all the information is correct. Does netsuite provide any kind of link for this type of a form or is it just the customer center that they would have to use?
