CSC Input Field Missing?

We just configured credit card processing to work with NetSuite.  When we go to the payment screen, the CSC input field which should be below the CC Number is not there.  (Though the "ignore CSC" checkbox is there).  The transactions do go through to Cybersource and process, but I'd prefer to enter the CSC when available.

Is there some configuration we are misssing to display the CSC input field?
