Unable to enter items today- IE & Firefox

This error pops up when trying to enter new items using a custom item-entry form. Error in both IE and Firefox in the latest browser versions using an Admin account. Errors with or without javascript on form.

Standard item form works OK, so this is obviously form-related after the maintenance fix from the weekend.

Anyone else seeing this?
