Left hand nav icons not showing up now

Hi,

Since 11.0.3 upgrade our image on the left hand navigation (our webdesigner made a custom one i think). Is not showing up and a big error box. Its such a pain coupled with customers not being able to checkout.

Can anyone let me know what to do about the image and make it appear again?
