Contact Form - Name Population

This problem just started this afternooon.  When modifying a contact's name on an existing contact record the static Name/ID field does not update with the changes.  I tested the field on both my custom form and the NS Standard Contact Form.  Both forms are NOT performing the Name/ID function of auto-filling to the contents inputted in the First and Last Name fields.

Is anyone else experiencing this problem?
