Custom Record permissions change after an upgrade

For many of my accounts, the default form preferences in Custom REcords go back to "Standard... form" after upgrades.  There is no particular pattern, it happens sometimes in some accounts and not in others.  And it can get really painful as I have scripts running on the custom forms!  I also notice that Labels on custom forms get defaulted back to the field names which is also quite irritating!

Does anyone else have this problem??
