Strange occurence with role permissions/history-

I have a really weird one-

All of my sales reps have the exact same role.

One of my sales reps can no longer create new transactions.  
I.E.  -Each time they go in to create a po in a blank screen, as soon as they populate the vendor field, the po pulls the last po created for that vendor, in the process before the PO is saved.  For example, a PO created last week, saved an issued to the vendor last week, comes up - exactly the same as the saved one- with the exception that the PO number
