Promo Codes Error Error and more errors

I am ready to pull my hair out with promo codes.
Every time i try and edit an existing promo code and add new items i get the same old - Record already exists error.
I even started a new promo code and entered only one matrix parent item and then all the children populated which was fine. The i hit edit and tried to a another matrix item and get error. Since there is only one other item in the promo code i know it does not exist...
What is wrong? Any ideas.
When adding matrix items should i add parents and children or only children or only parents.
