Price level defaulting wrong on web orders

All of our web store orders come in with a price level that is incorrect.  The actual price IS correct but the name of the price level is completely incorrect.  The price level that is defaulting has a totally different pricing structure than what is coming through too.

Anyone know why this is happening?  It's affecting my reporting integrity.....
