Duplicate Detection Warning No Longer Working?

We setup duplicate detection a long time ago and have not made any changes to it.  Customer Duplicates are based on Address 1.  We no longer see a warning on a customer record when a duplicate Customer Record exists.  I can verify this by doing a search.

Due to the size of our database, we have over 2,800 groups of (potential) duplicate customers under Mass Duplicate Record Merge.  There's no way for us to manage dupicates that way, so we have to be able to identify them at the customer / contact level.

What happened?  Anyone else have this issue?
