Order Items Screen Malfunction

I just wanted to ask if anyone else has had this issue in the past using the "order items" screen.
This morning our purchaser used the screen and was creating a PO for all items needed for a particular vendor, Holloway.
When he hit submit, NetSuite generated purchase orders for every item from all vendors on the page!
This created about 30-35 purchase orders that he had to go back and manually delete.

Tried to use the screen again after manually deleting and it happened again!
This happened once earlier to us about 6 months ago.

Please, has anyone ever heard about this bug? I can't submit a case because I won't be able to show netsuite support what happens - classic, "unable to reproduce the issue".
