Netsuite generating erroneous and duplicate System Notes history on items

Lists > Items > Inventory Item (any item)

In the History tab, System Notes sub tab we have many many many history errors !!!

tens of thousands of errors generated each night. (estimated since we have 50,000+ items all generating the same type of erroneous history).
