System notes not showing all changes!

I have found that we still do not get to see all the changes that happen on a customer.
In this case, the child of field data is not being recorded when changes occur.
This was only brought to my attention by accounts when they found a dicrepency when they ran a statment for a customer and it did not match. It was found that someone had added a store as a child to the above, when teh statment was run it actually showed both accounts on the parent statement without indicating the different stores.
