System Notes showing changes not made by us

There have been several instances recently where changes have been made to transactions that were not made by anyone on our staff:

1. April 30 transactions (Inventory Adjustments, fulfilled and invoiced Sales Orders, etc.): The original transactions took place days or weeks earlier, but all of sudden on 4/30, at the same time of day, changes were made. The name under Set by is a former employee who has not had access to NetSuite since December 2007. I turned in an issue on this more than a month ago and although it was sent to the QA team as "urgent" I haven't heard anything and the issue is still not fixed. (Case #713943)
