PDFs not displaying png file correctly

I've got a really weird problem going on and it's driving me nuts!

I have setup a png of our company logo for html and pdf forms.  The resolution is 96dpi and the physical dimensions are 1.5" x 1"
I created the image from Adobe Image Ready CS2 (about as mainstream as you can get).

The images look fine on my HTML templates but in PDFs they are scaled up about 150% and pixelated.  The size settings on the PDF layout editor appears to have no effect at all.

I can set it to 1" or 7" and the logo is the same exact size.  I have triple checked that I'm editing the correct layout and form.
