Can't edit Cash Refund - anyone else?

I've created a Cash Refund and saved no problem.  I then edited it and changed a field, ah hell, let's say it was the the memo field!  So then I tried to save it and...

This record already exists


then just below that

Just for fun I edited and saved without changing anything and I get the same result.
