situation with: Online Customer Form

I can´t add new Online Customer Forms

when I try to save modifications or save a new ONLINE CUSTOMER FORM 
it send me this NOTICE:

You are missing the following required field(s):

CUSTENTITYSATISFACCION
--------------------------------------------------


all this trough the Form Type: Default Form Template


the word: "CUSTENTITYSATISFACCION" it's a word that I suppose someone on my company add as a CUSTOM RECORDS, because I try to find it on the GLOBAL SEARCH on NETSUITE and NETSUITE HELP CENTER too and find nothing about this word


can someone help me how to fix this situation?
