Online Customer Form "Error: Unexpected Error"

Hey all,

I have setup an Online Customer Form. When I hit submit, the result is "Error: An unexpected error has occurred. Please go back and try again." Has anyone experienced this error? I'm not doing anything fancy here. Just a simple public Online Customer Form being submitted.
