Website Case Form - Unexpected Error

We have a Request a Quote form on each item page on our website, which is a custom front-end to a Netsuite Case form. The last time this page was changed was in June. In the last few days, users who submit the form are OFTEN getting a Netsuite error message:

"An unexpected error has occurred. Technical Support has been alerted to this problem."

The Netsuite page source code shows an error comment:
"It appears that the URL for this page got cut off or garbled in your mail client. Please copy the full URL and paste it into the address field of your browser."
