User Event intermittent

I have a custom record and a user event script with beforeload enabled. I have NO event type selected in the deployment and run as admin and all roles set for permissions:

1. View list of records: Click 'Edit' next to a record (User Event does NOT fire).

2. View list of records: Click 'View' next to a record (User Event does NOT fire).
2a. Click the 'Edit' button from record being viewed (User Event DOES fire)

3. View list of records: Click the 'New' button in lower right (User Event DOES fire).

What the heck is going on?
